The following is a list of acceptable forms of credentials identifying you as
a member of the professional beauty industry:
• Cosmetology license
• International cosmetology license
• Salon/Spa business license denoting salon ownership
• Business card from a legitimate salon with your name imprinted on it
• Letter on Company Letterhead from owner of salon/spa denoting employment
• W-2 form with Salon/Spa/Distributor Name
• Invoice from professional beauty product distributor/manufacturer
• Cosmetology school business or state license
• Professional beauty association membership card (i.e., NCA, CCA, TSA, ISPA, etc.)
• Certificate of graduation
• Invoice showing paid classes for last semester
• Letter on School Letterhead from teacher or school director indicating their graduation
• Invoice showing paid application for state license
• CosmoProf® PROCLUB® Card
• Distributor Card
• Cosmetology School Time Sheet or report card
• Cosmetology School ID card
• Student ID or letter from Certified Instructor on school letterhead
• Other appropriate license(s) denoting you as a member of the professional beauty industry
